Collection 1000395 (herbarium acc. no. FLAS-F-22488) is Hebeloma sordidulum. It is the holotype of heterotypic synonym Hebeloma levyanum Murrill (1946) ["1945"].

    Collection 1000395 was collected by W.A. Murrill, B. Watson on 14th January 1940 in Levy County, Gulf Hammock , Florida, UNITED STATES (approx. 29.2527°N, 82.7234°W, altitude approximately 0 m above sea level - Google maps). No ectomycorrhizal plants were recorded.

    The surrounding habitat was described as not recorded.
