Collection 1000241 (herbarium acc. no. PRC B380) is Hebeloma crustuliniforme. It is the lectotype of heterotypic synonym Hebeloma macrosporum Velen. (1920).

    Collection 1000241 was collected by J. Velenovsky in August 1915 in Mnichovice, CZECH REPUBLIC (49.5612°N, 14.4245°E, altitude approximately 590 m above sea level - Google maps). No ectomycorrhizal plants were recorded.

    The surrounding habitat was described as not recorded.
